First Visit
When you arrive, we’ll give you the necessary paperwork to fill out. You can also download these forms below and bring them in completed. You’ll sign in, and we’ll get a copy of your insurance and identification cards. Then, you’ll sit down with Dr. Dawn, who’ll listen carefully to what’s brought you in and how your life has been affected by your issue. We’ll talk through your history, then do a focused examination to determine the source of your problem. Our findings will be explained to you, and then treatment will be performed.
We’ll discuss a plan that’s appropriate for you and give you recommendations for home care and what to expect after your first chiropractic adjustment. Please plan on spending about 60 -90 minutes with us.

Insurance

No insurance
For patients that don't have chiropractic insurance coverage, we offer flexible payment plans that can work within your budget. After all, your health should always come first! Patients that pay out of pocket, find chiropractic to be surprisingly affordable.
Medicare
If you are Medicare eligible, we will submit a receipt for your services to Medicare for you. Call to schedule your complimentary consultation with Dr. Dawn today!

Q: Does chiropractic treatment require a referral from an MD?
A: A referral is usually not needed to see a doctor of chiropractic (DC); however, your health plan may have specific referral requirements. You may want to contact your employer's human resources department - or the insurance plan directly - to find out if there are any referral requirements. Most plans allow you to just call and schedule an appointment with a DC.
Q: Do Insurance plans cover chiropractic?
A: Yes. Chiropractic care is included in most health insurance plans, including major medical plans, workers' compensation, Medicare, some Medicaid plans, and Blue Cross Blue Shield plans for federal employees, among others. Chiropractic care is also available to active duty members.
